AZ COMMON GROUND SERVICES

Our goal is to provide reentry services to medium to high-risk youth and adults once released from incarceration back to Maricopa County. We utilize evidence-based practices (EBP) to reduce recidivism rates by providing all individuals who are in our program ongoing case management, educational programming, mentoring, and workforce development.

 

Every client that comes to our organization starts by attending our orientation and completing an application. Many clients attend our orientation for immediate resources such as bus passes, interview clothes, food assistance, housing assistance, and job leads.

 

When clients are accepted into our Prison Reentry Program, they complete the intake process by being assessed using the LS/CMI (Level of Service Case Management Inventory) to measure their risk levels, their needs assessment, and case management plan. 

BECOME A PART OF THE JOURNEY

Each potential client will be selected following our orientation process. Following acceptance, each client moves forward within the application and intake phase. Each client will be assessed using our EBP case management tool, Level of Service/Case Management Inventory (LS/CMI) is an assessment that measures the risk and needs factors of late adolescent and adult offenders. The LS/CMI is also a fully functioning case management tool. This single application provides all the essential tools needed to aid professionals in the treatment planning and management of offenders in correctional, justice, forensic, prevention and related agencies.
